For some partial and full-thickness supraspinatus (rotator cuff) tears, ultrasound-guided injections of bone marrow concentrate (BMC), a stem cell therapy, combined with platelet-rich plasma (PRP) may reduce pain and improve shoulder function. Emerging research suggests these injections can outperform exercise therapy alone in the short term, though diagnosis and injection technique still decide the result.

What Does the Best Current Research Say About BMC and PRP for Rotator Cuff Tears?
The best current research on BMC and PRP for rotator cuff tears is a 2024 randomized controlled trial that followed 51 patients with a torn supraspinatus tendon. Patients treated with bone marrow concentrate plus platelet products reported significantly greater improvement in shoulder function, pain, and overall recovery at three months than patients doing exercise therapy alone (Centeno et al., BMC Musculoskeletal Disorders, 2024), and those results were maintained through two years. Among patients with before-and-after MRIs, follow-up imaging demonstrated healing and reduced tear size in 73% of shoulders. Additionally, no serious adverse events were reported in any patients.
These are meaningful, sustained results from a randomized trial. It is one modest-sized study, and it focused on non-retracted tears, so that is where the evidence is strongest. BMC and PRP are not FDA-approved treatments for rotator cuff tears, and because they use your own tissue, results vary from person to person.
Our own patient outcomes point in the same direction. In our registry, 74% of patients treated for partial-thickness tendon tears report significant pain relief. Among shoulder patients, 82% report relief by 18 months (data updated July 2026).
Do BMC and PRP Injections Replace Rehab, or Help You Avoid Surgery?
BMC and PRP injections do not replace rehab, and for the right tear they may help you avoid surgery. Loading and rehabilitation remain foundational for rotator cuff pain, and these injections are meant to work alongside a structured rehab plan.
Surgery is still the right call for some tears, particularly large retracted ones. The trial was not designed to prove that injections prevent surgery, but the numbers are encouraging: only about 1 in 50 patients, after a re-injury, went on to rotator cuff surgery during the two-year follow-up, even though most of the enrolled tears would have been considered surgical candidates (Centeno et al., BMC Musculoskeletal Disorders, 2024).
What Decides Whether BMC and PRP Injections Help Your Rotator Cuff Tear?
The biggest factor is an accurate diagnosis of which structures are actually driving your shoulder pain. A rotator cuff finding on imaging may not be the source of your symptoms, which is why a hands-on exam and diagnostic imaging come before any injection is discussed. Once the diagnosis is clear, the platelet and cell dose and the accuracy of ultrasound-guided placement influence how much benefit these injections can offer.
