Privacy Policy
Effective Date: April 10, 2026 • Last Updated: April 10, 2026
Your privacy matters to us. This Privacy Policy explains what information we collect through regenerativeperformance.com (the “Site”), how we use it, and the choices you have. This policy covers the Site only. Information collected during in-person or telehealth care is Protected Health Information under HIPAA and is governed by our separate Notice of Privacy Practices, available at the clinic or on request.
Information We Collect
On some pages of our Site you can request information, download a guide, take a candidacy quiz, request a quote, or schedule a consultation. The information we collect on those pages may include:
- Name
- Email address
- Phone number
- Mailing address (when requested)
- Information about a physical condition or symptom you are experiencing
- Payment information (processed by a secure third-party payment processor; we do not store card numbers)
After your initial submission, we may request additional information by phone, email, or through a secure form so we can respond accurately to your request.
How We Use Information
We use the information you submit to:
- Respond to your inquiry and answer questions about our services.
- Schedule your consultation, appointment, or procedure.
- Send appointment confirmations, reminders, and related administrative messages.
- Send occasional marketing emails and, if you have opted in, SMS messages.
- Improve our Site and services.
- Meet legal, tax, accounting, and regulatory obligations.
We do not sell, rent, or share your personal information with third parties for their marketing purposes. We never trade access to our patient or subscriber lists.
Sharing of Information
We share limited information only with service providers who help us operate the Site and our practice (for example, our website host, email platform, scheduling tool, and payment processor). These providers are contractually required to protect your information and use it only to provide services to us.
We may also disclose information when required by law, in response to a subpoena or court order, to protect the rights or safety of Regenerative Performance or others, or in connection with a business transaction (such as a merger or asset sale), with notice to you.
Web Data and Analytics
Like most websites, we automatically collect non-identifying information about your visit, including IP address, browser type, referring website, pages viewed, and approximate location. We use this aggregate data through Google Analytics 4 to understand how our Site is used and improve the experience. We do not use this data to identify you personally.
We do not run behavioral advertising pixels on pages that describe specific medical conditions or symptoms. We rely on consent-based audience matching — if you subscribe to our newsletter, download a guide, or fill out a form, we may use your submitted email to match you as part of advertising audiences on platforms like Facebook, Instagram, or Google. You control this by unsubscribing at any time or contacting us to remove your information.
Email and Text Messaging
When you provide your email or phone number through a form, you consent to receive responses from our team, appointment-related communication, and occasional marketing messages. Every marketing email includes an unsubscribe link. To stop SMS messages, reply STOP to any text you receive from us. Message and data rates may apply. We do not share SMS opt-in consent or phone numbers with any third party for their marketing purposes.
Your Choices
You have the right to:
- Access the personal information we hold about you.
- Correct inaccurate information.
- Delete information we hold about you, subject to any legal record-keeping requirements.
- Opt out of marketing emails or SMS at any time.
- Opt out of having your email used for ad audience matching on Meta or Google platforms.
To make any of these requests, email office@regenerativeperformance.com or call (480) 508-4226. We will verify your identity and respond within 30 days.
If your request concerns medical records or health information collected during care, those requests are handled under HIPAA through our Notice of Privacy Practices, not this policy.
Data Security
We maintain reasonable administrative, technical, and physical safeguards to protect your information, including encryption in transit, access controls, and vendor review. If we ever become aware of a breach affecting your personal information, we will notify you and applicable regulators as required by Arizona law, HIPAA, and any other applicable statutes.
Children
This Site is not directed to children under 13 and we do not knowingly collect personal information from children under 13. Care for minor patients is handled in the clinic with verified parental consent, not through public web forms.
Changes to This Policy
We may update this Privacy Policy from time to time. When we do, we will update the “Last Updated” date at the top. For material changes, we will provide additional notice by email or a notice on the Site.
